Childbirth and Childcare Lump-Sum Grant

The Childbirth and Childcare Lump-Sum Grant is paid when an insured person or an insurance dependent family member gives birth.
Procedures to add the newborn as a dependent are also required, where applicable.

Request a Childbirth and Childcare Lump-Sum Grant

When using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions

Submit the documents on agreement to use the system to the medical care institution or other facility where you plan to give birth. (You need not take any action with respect to the Health Insurance Association. For details, check with the medical care institution or other facility where you plan to give birth.)

If you paid the entire amount of childbirth expenses to the medical care institution

Submit the following application to the Health Insurance Association if you have not used the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf or you have given birth overseas.

Required documents:

  • Copies of agreements from the medical care institution or other facility
    (documents demonstrating that you have not concluded a proxy agreement with the medical care institution or other facility on use of the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions and indicating the name of the Health Insurance Association as application recipient)
  • Certification from a doctor (midwife), certificate of receipt of registration of birth issued by mayor of municipality (copies not accepted), or copy of family register (photocopies not accepted)
  • Copies of receipts for and details of childbirth expenses
    (documents demonstrating that the details are the same as those on the special invoice submitted by the medical care institution or other facility to the Health Insurance Society and, if giving birth at an institution that is a member of the maternity medical care compensation scheme, showing printed or stamped text certifying that the childbirth took place at a member institution)
Deadline: As soon as possible
Notes on filling out this form: Submit this application only if you were unable to use the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or if the birth took place overseas.
The application cannot be accepted without certification from a doctor (midwife) or a certificate issued by the mayor of your municipality. (Copies of the maternal and child health handbook are not acceptable.)
Birth overseas In the case of a birth overseas, no copies of agreements from and receipts for and details of childbirth expenses are required. If there is no certification on the application form, attach a copy of the family register or a birth certificate issued by the medical institution or national (or local) government.

When using the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf

If you would like to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf, submit the following application to the Health Insurance Association in advance.

Required documents: Application for Payment of Childbirth and Childcare Lump-sum Grant (for Receipt on Your Behalf)
Check with the Health Insurance Association.
Deadline: In advance

Add a child as an insurance dependent.

Conduct separate procedures to have the child named as an insurance dependent.
